Full Job Description
Co-ordinating the delivery of cardiac rehabilitation in association with the multi-disciplinary team.
Providing support and care for cardiac patients who have suffered an acute cardiac event.
To provide phase I cardiac rehabilitation in the hospital and to provide continuity of care for patients post discharge.
To provide phase II and III cardiac rehabilitation in an out-patient setting in collaboration with the multi-disciplinary team
To liaise with primary and secondary health care teams and other agencies.
Key duties inculde:
- To triage referrals to the cardiac rehab team, identifying appropriate patient and the best pathway for care.
- To visit patients who have suffered an acute cardiac event.
- To provide information to patients and their families on treatment, recovery, rehabilitation and secondary prevention.
- To organise and run cardiac rehabilitation assessment clinic to assess the patient’s physical and psychological state.
- To offer support with lifestyle modification such as smoking cessation and dietary changes and refer onto other specialists when appropriate.
