Band 7 Senior Practice Nurse

Duties and responsibilities of the role include but are not limited to: Providing safe, evidence-based, cost-effective, and individualised patient care. Working autonomously using advanced levels of history taking and examination skills within multi-disciplinary team, guidelines, and protocols. Utilise advanced clinical skills in assessment and/or formulate a diagnosis and/or treatment plan. Leading with the care of patients with chronic illnesses e.g., Asthma, Family Planning, Diabetes, Chronic Heart Disease (CHD) and hypertension ensuring continued maintenance of the Chronic Disease Register.

Leading and promoting health and well-being clinics using health promotion, health education, screening, and therapeutic communication skills (i.e., smoking cessation, weight management, alcohol misuse, sexual health, and cervical screening). Having direct responsibility for patient caseload, ensuring all patients have an accurate plan of care, which reflects the assessment undertaken and incorporate the issues and recommendations made. The sift will be completed within 5 working days of the vacancy closing date. Interviews will be held on a date to be confirmed. There will be no reimbursement for any travel expenses for the interview. There will be a 10 minute presentation ”What will be your key priorities in the first 6 months and how do you plan to achieve these?”.
