Memory Nurse

As a Memory Nurse with the Barnet Memory Service, participate as a full member of the multi-disciplinary memory service. Being responsible for co-ordinating the service user’s journey through the memory clinic. Play a key role in preparing the service user and carer for the diagnostic process and seeking consent from the service user regarding further assessment. Being responsible for supporting the full baseline assessment and completing the collateral history with the carer, along with providing ongoing information and support to the service user and carer throughout the length of the assessment.

To support initial assessments and reviews of patients referred to the memory clinic, including risk assessments, completing assessment scales and associated social care needs.

To undertake reviews/follow ups/appointments to monitor cognition and medication.

Liaise directly with GPs to ensure appropriate handover of care after review

Provide carer with support and information on medication & its management, educating both patients & carers to understand the illness & its prognosis.
