Memory Nurse
Full Job Description
To support initial assessments and reviews of patients referred to the memory clinic, including risk assessments, completing assessment scales and associated social care needs.
To undertake reviews/follow ups/appointments to monitor cognition and medication.
Liaise directly with GPs to ensure appropriate handover of care after review
Provide carer with support and information on medication & its management, educating both patients & carers to understand the illness & its prognosis.
