Your Opportunity:

Alberta Health Services has an exciting opening for a Nurse Practitioner to work in our Home Nursing Team. This is a “pioneering” role with an opportunity to develop and implement new policies and programs related to in-home care, advanced care planning, frailty, transitions and placements. You will conduct comprehensive assessments and provide consultative services to thirteen physicians. As a leader and mentor for the team, you will assist with the more unique and complex transitions from Acute Care to home or residential care. You will participate on a Nurse Practitioner Advisory Committee and collaborate with Nurse Practitioners in Grande Prairie and our Palliative Care team. This is a vital and diverse role. At times, you may even provide primary care or help patients navigate their finances or social matters. You will have a community of practice to support you and research opportunities. Description:

Your work will be based out of northern and primarily rural communities in Alberta. This position will be expected to participate in an on-call rotation to provide 24/7 telephone consult coverage throughout North Zone. Responsibilities will include placement support, advanced care planning, cognitive screening, delirium screening, lodge rounds, incontinence assessments, and home visits in complex scenarios.

Required Qualifications:Minimum of 3 years’ of recent clinical experience as a NP, with at least 1 year experience working with older adult population. CNA certification in geriatrics. Evidence of knowledge of Continuing Care service delivery options, Designated Living Option (DLO) assessment/placement criteria, and community supports and agencies. Ability to adapt skills to a virtual working environment. Proven experience of successful teaching. Proven experience in program development. Demonstrated ability to function collaboratively and effectively as an inter-professional team member. Exhibit strong time management and prioritization skills, with a demonstrated ability to function effectively both independently and within a dynamic team environment. Successful completion of an approved Nurse Practitioner program at a minimum of a Master of Nursing level or equivalent. Active or eligible for registration as a Nurse Practitioner with the College of Registered Nurses of Alberta with a practice permit indicating appropriate stream of practice. Experience within Continuing Care streams of Home Care, Designated Supportive Living, and/or Long Term Care. Willingness to complete additional education and/or certifications specific to role.


Additional Required Qualifications:
Demonstrated ability to provide support and advice to Continuing Care teams including but not limited to care planning, assessment, care interventions, and development of programs for CC clients with complex cognitive/mental health/behavioral concerns, palliative care needs, complexities of chronic illness, co-morbidities and frailty, for example. Demonstrated ability to actively integrate Client and Family-Centered-Care principles into practice. Excellent communication and interpersonal skills in order to work collaboratively with members of the inter-professional team. Proven experience in critical thinking, conflict resolution and problem-solving skills. Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP).
