TELUS Health – Care Centres is looking for a Nurse Practitioner (NP) to join our dynamic Edmonton team! TELUS Health Care Centres are committed to innovation, collaboration and excellence. Nurse Practitioners are responsible for the provision of high quality care services for the THCC Family Health Team (FHT), and play a key role in achieving its vision and mission.
As part of the team, you’ll be responsible for the provision of family health nursing care to the population of patients seen in the family practice service and requires a thorough knowledge of primary care nursing within the scope of the Nurse Practitioner as determined by the College of Registered Nurses of Alberta.
What you’ll do
Our Nurse Practitioners complete medical assessments and procedures, health promotion, disease management, and risk reduction strategies. Nurse Practitioners have a patient family centered focus with a holistic approach to patient care.
You work closely with a multi-disciplinary team to ensure clients, their families and communities are supported. At TELUS Health Care Centres, the Nurse Practitioner role has traditionally focused on the children, adolescent and young adult populations, while still supporting clients of all ages. Together, we work as a Family Health Team (FHR) and you will play a key role in achieving its vision and mission. The role also requires a high level of interpersonal skills to interact with patients and their families in a holistic and compassionate manner.
Day-to-day responsibilities include:
- Individual patient assessment and treatment – health assessment, immunizations, ordering laboratory tests, diagnosis, treating illness, prescribing medications, procedures, regular patient follow up to promote health and counsel on disease prevention
- Creating patient plans in follow up to assessments
- Making internal and external referrals as appropriate
- Coordinating appropriate diagnostic tests and procedures independently and/or in consultation with family physicians
- Participating as a clinical team member for complex and specialized care, including but not limited to, prenatal care, chronic disease, child and young adult health and wellness
- Engaging in networking, conferences, and presentations both as an ambassador of TELUS Health and as a knowledgeable clinician
- Maintaining strong knowledge of community resources in order to provide the patient with information and care resources outside of TELUS Health Care Centres
Qualifications
What you bring:
- Masters of Nursing – Nurse Practitioner in Primary Care, post-masters diploma
- CRNA registration in good standing
- CPR
- Knowledge and experience in primary care
- Current experience in chronic disease management preferred
- Advanced clinical nursing skills within the scope of a FNP; additional training relevant to practice is an asset
- Skilled in program planning, implementation and evaluation
- Ability to take initiative and demonstrate proactive/preventative care
- Exceptional organizational and personal skills, positive and results focused attitude, and excellent written and oral communication skills
- Computer competency and familiarity with an EMR
