Healthy Family Teams are part of the Healthy Families Programme, delivering an integrated 0-19 public health nursing service for children, young people and their families.Advice, care, and interventions are provided by the Specialist Public Health Practitioners (Health Visitors andSchool Nurses) and their multidisciplinary skill mixed teams offering a joined-up service to all families in Nottinghamshire. There are 20 locally based Healthy Family Teams across the County who deliver the programme.The service is provided to any family living in Nottinghamshire with a child between 0-19 years old, expectant mums and for young people attending Nottinghamshire schools up to the age of 19 years.The service works 8.30am 5pm Monday to Friday but can also work out of hours if required. Each team of practitioners and support staff have a responsibility for a geographical area and work across the whole 0-19 years age range making sure the care provided is delivered by the right team member at the right time for the child, young person and their family. The post holder will be expected to: Manage their discipline specific team at base, working to meet the needs of the resident population including clients with continuing healthcare needs, the promotion of self-care and health and wellbeing or work in a capacity to have clinical input.To supervise, develop and support other professional colleagues and staffTo assess and monitor competency in order to maintain quality servicesTo teach qualified and unqualified staffTo work with managers in the development of services to meet the changing needs of the resident population, including the response to professional issues as they ariseTo work with the Higher Educational Institutions (HEI) and the organisation in the practical development of nurses undertaking Post Graduate education, assessing and facilitating learning inorder to attain professional competence through experience in practice.Provide learning opportunities that are underpinned by evidence-based practice and linked to those of the HEI, other educational providers and the employing organisations.To promote practice development in order to raise professional standards in line with Government policy and the changing needs of the NHS For an informal discussion regarding this post please contact Elizabeth Henson on 07769 367161
