Virtual Ward Modern Matron

Direct Management of the virtual ward beds to ensure that we are maximising and monitoring the agreed bed capacity in each geopgraphical area at any given time. Taking the Lead, in working with referring services and the Unscheduled Care Co-ordination Centre in making decisions on accepting or declining patients onto the Virtual Ward. Responsible for the case management and allocation of patients to the Virtual Ward clinical staffing that have been referred through to the Virtual Ward based on the level of acuity of the patient. Liaising and engaging with key professionals, including Acute Medics, Senior Managers, Senior Clinicians, GPs, Community Senior Managers and Local Authority Leads to ensure that all are working to the agreed operating pathways for the virtual ward and achieving the same goal of patient clinical safety, admission avoidance and supported early discharge where possible.
