With the support of the respiratory virtual ward lead, to develop and evaluate a respiratory virtual ward within University Hospital Sussex West (UHS) in Worthing and Chichester, and to assist in the planning and progression of service provision To be in charge of the ordering of the necessary equipment required to deliver and monitor this service, within the financial budget for this service. To act as a highly specialised source of expertise on the management and provision of respiratory virtual ward care within Worthing and Chichester As an autonomous practitioner, use advanced skills in the assessment and treatment of adults with potentially complex respiratory conditions within hospital to identify individuals for treatment at home. To work with the community teams (across Worthing and Chichester) to identify individual patients with known respiratory disease in the community who would be suitable for Respiratory Virtual ward admission as part of hospital admission avoidance. To offer assessment and treatment (including point-of-care blood monitoring) in patients homes and interpret remote monitoring physiological parameters to decide which patients need home visits versus ongoing remote consultations To, where necessary, work with the community virtual ward pharmacy team to ensure all required medications are available for individuals within the respiratory virtual ward To work with the hospital at home team and act as a bridge between the acute trust and the patients home in delivery quality and safe healthcare.

To supervise, evaluate and assess the performance of other members of the team and those of any students on rotational placements. To maintain own Continued Professional Development (CPD) To provide highly specialist advice and training to physiotherapists, nurses, doctors and students across the trust.
